Internship Logbooks and Institutional Internship Evaluations for Students Interning in the 2025-2026 Academic Year
Students who will be doing internships in the 2025-2026 academic year are required to print the following Student Internship Logbook and submit it as their internship logbook.
For institutional internship evaluations, students must also have the Internship Evaluation Form and the Intern Student Attendance Tracking Chart completed and approved in one copy by the relevant institution's internship officer and submitted to our school in a CONFIDENTIAL, SEALED ENVELOPE.
While the CONFIDENTIAL, SEALED ENVELOPE procedure continues for internship logbooks and institutional evaluations for students interning under the National Internship Program, the procedures specified in the guidelines will be carried out through the National Internship Program platform.
NOTE: The 4th page (the blank page) in the Student Internship Logbook will be cut out for each internship day. For example, if the internship period is 20 working days, you must cut out at least 20 blank pages from the 4th page.
